A
45-year-old man presented with one day onset of severe
right-sided occipital headache. His headache was progressively
worse and then developed severe vomiting a few hours earlier.
His physical examination was unremarkable.
Imaging
study
Fig.
1 Noncontrast CT scan
CT Findings:
- Normal brain parenchyma and ventricular system
- Hyperdense
right transverse sinus
Pitfalls: 1. Hyperdense venous sinuses may be due to high hematocrit.
2. Hyperdense and enlarged venous sinus suggest venous
sinus thrombosis.
3. If venous sinus thrombosis is suspicious,
contrast study should be proceeded. On contrast-enhanced
CT scan, the thrombus does not enhance while the venous
blood enhance.
Comments:
- In this case, although enlarged right transverse sinus
may be due to dominant right transverse sinus but the hyperdense
right transverse sinus is denser than both the cavernous
sinuses and the middle cerebral arteries. Venous sinus
thrombosis is considered.
