Story 1:  Urinary catheterization

 

 

It is a sterile Foley catheter insertion within the bladder, in order to drain urine

 

Who are treated?

 

 

 

 

    1.  There is bladder outlet obstruction.

 

    1.  In a case of measuring the urine amount, to assess renal function such as while patients in shock.

 

    1.  Those with neurological controlling problems in urinary excretion.

 

  1.  For patients with an operation; providing before the operation, during operation, and after the operation.

 

 

Foley catheter  care

 

1.  No folding or bending and  attach to the patient with plaster.

 

2. The urine bag should be placed lower than the bladder.

 

3. Maintain the urinary system as a closed system.

 

4. Clean catheter in the morning and evening, involving after defecation, the doctor will assess the need for urinary catheter use every day.

 

 

 

 

 

 

Story 2: Physical Restraints

 

Restrain refers to limiting the body movement of patients from violent behavior and not being cooperative in the treatment. In order to  prevent potential harm to patients.

 

 

 

Who should be provided?

 

    1.  In a case of patients with brain pathology or pathology, which is derivative from some diseases

 

    1. Postoperative patients who are not recovered from anesthetics procedure.

 

    1. Patients who have unstable mental states.

 

    1. Patients who are uncooperative in treatment

 

Principles of physical restraint.

 

1. Use some equipment which has been prepared tie the patient, then follow up a tight site in every 1-2 hours.

2. Measure a pulse step and lean the body of the patient in every 1-2 hours.

3. Not being strained during family visiting but always be away of.

4. Discontinue the restraint when patients cooperate with.

 

 

Story 3: Identify

Story 4: Patient with Intubation Care

 

1. Patients will be evaluated for extubation.

 

2.  Set up a head position at 30-45 degrees, brushing patient’s teeth and cleaning mouth with 0.12% of chiohexidine.

 

3. provide mucus suction machine with sterile technique and gentle

 

4. Prevent the patient from pulling the equipment, do not let it move and slip.
