Today, I will talk about my mother’s experience with pelvic organ prolapse. Some people might be a bit confused, but from my conversations with doctors, this condition is known by different names among patients. Some call it diaphragm prolapse, vaginal prolapse, or sagging. What I am about to share is a very private matter for women, so I will keep my mother’s name confidential. Even though she is 88 years old, issues related to women’s intimate areas are always very personal regardless of age.
 

A message from a loved one 
My mother is 88 years old. First, I need to go back a few years. She had pelvic organ prolapse once before. At that time, her uterus slightly protruded outside her body. She saw a doctor and was advised to have surgery, but she was afraid of surgery—actually, she was quite afraid of doctors in general—so she refused the operation. About 6-7 years ago, she had a severe cough combined with an infection, so she had to be treated at the hospital. At that time, she was treated for pulmonary edema and heart disease and was continuously monitored. Even at 88 years old, my mother can still take care of small tasks by herself, such as warming food, walking, and using the bathroom.
 
In June 2017, I visited my mother at her home in Samut Songkhram province. She called me into the bathroom to show me the abnormality. I was quite shocked because what was hanging out was large, sausage-shaped, about the size of a packaged liver spread, and red as if inflamed. It could not retract back inside naturally like it did years ago.
 

Delicate care 
On the same day, I took my mother from Samut Songkhram to the Women’s Health Center at Phyathai 3 Hospital. However, there was no female doctor available. Since my mother was elderly and uncomfortable being examined by a male doctor, we scheduled an appointment with a female doctor the next morning. After a thorough examination, we were advised to consult about surgery with Assistant Professor Dr. Pichai Leerasiri, Obstetrician-Gynecologist, Subspecialist in Reproductive Medicine
From the discussion, we learned that this condition has several stages based on symptoms. My mother had stage 4 pelvic organ prolapse, with the organ protruding outside the body. After consulting with the doctor and deciding on surgery, it was not possible to operate immediately because the hospital needed to conduct a detailed physical examination by specialists in various fields to ensure readiness. Given my mother’s advanced age, extra caution was necessary. Finally, it was decided that she could undergo surgery. Dr. Pichai and the medical team planned and prepared thoroughly for the operation, which was very successful. After surgery, she needed to be monitored for urinary system care for a while.
 
Regarding the cause of my mother’s pelvic organ prolapse, it is difficult to pinpoint exactly because it may result from multiple factors. Known risk factors for pelvic organ prolapse include age—older age increases the risk, and my mother is 88 years old, so naturally, the risk is higher. Pregnancy—especially for women with multiple children and vaginal deliveries, the risk increases with the number of births. My mother has three children and gave birth naturally in the traditional way. Other factors include chronic coughing and regularly lifting heavy objects, which may relate to my mother’s history. She was treated for chronic cough in the past, and during her working years, she was a coconut farmer who regularly lifted coconuts herself. Each of these factors likely contributed to her pelvic organ prolapse today.

However, what I want to communicate to all women is that whenever your body shows abnormalities, especially in intimate areas, you should promptly seek examination and treatment. In my mother’s case, if she had surgery when she was younger and healthier, her recovery would have been faster, and the prolapse would not have protruded as much as it did. If the patient is an elderly mother under the care of her children, even if she refuses treatment, you should not give in but insist on her receiving care because ultimately, the one who benefits from the treatment is the beloved mother herself.
