?Miscarriage? and ?Preterm Labor? are things that can happen when a mother is pregnant… The ?Phatthawee Suk? family shares their experience to help parents prepare to handle crises that may occur during pregnancy.
Mr. Suraphat Phatthawee Suk, 30 years old (husband), and Mrs. Nattha Phatthawee Suk, 30 years old (wife) came for their first prenatal checkup at Phyathai 3 Hospital after being married for 5 years.
Mr. Suraphat said that he and his wife chose Phyathai 3 Hospital because there are specialists who take care of the fetus. They had prenatal care with Dr. Adisorn Aksornphusitpong, an obstetrician specializing in maternal-fetal medicine.
The Beginning Journey They started prenatal care with the doctor at 6 weeks. At 7 weeks, the wife had vaginal bleeding. The doctor treated and assessed the risk and found no abnormalities but gave an injection to prevent miscarriage. They also did the NIFTY TEST, which came back normal. Normally, my wife is slim and small (weighing about 35 kg), and during the 3rd month, she had severe morning sickness, almost allergic to all proteins and could hardly eat, causing further weight loss. We were very worried, but the doctor explained everything in detail, which made us feel more at ease.
Don’t think it’s (just) because a big problem is coming At 14 weeks, there was vaginal bleeding again. This time the doctor gave 3 injections to prevent miscarriage. But the big problem started at 21 weeks when my wife had diarrhea from drinking milk. Symptoms began in the afternoon and did not improve by evening, so we went to a nearby hospital thinking it was just regular diarrhea. But it wasn’t! They found abnormalities, so we took my wife to Phyathai 3 Hospital and found that her abdomen was hard and she had symptoms of preterm labor. The cervix was thin, like someone close to delivery. We were shocked because if the baby came out now, it would be a miscarriage.
The doctor treated her by placing a silicone cerclage to strengthen the cervix and gave intravenous medication to stop preterm contractions. She was advised to rest and be monitored at the hospital until delivery. Eventually, she had to be transferred to a hospital with the right treatment rights, but the situation worsened because the delivery waiting room did not allow visitors. My wife had to face all the problems alone during her stay. We could only call her by phone. Moreover, she had to listen to the cries of other patients in labor waiting to deliver, which made her feel even more depressed. As time passed, her mental state deteriorated, and she developed depression. At 24 weeks, she had a vaginal infection and had to stay longer in the hospital. This was also when the doctor started giving steroid injections to stimulate the baby’s lungs, although the baby weighed only 500 grams.
After being discharged from the hospital with treatment rights at 26 weeks, we hurried to have a detailed checkup at Phyathai 3 Hospital. We found that the baby’s weight was below standard and the cervix was dilated 3 cm, which is equal to the cerclage size. This means if the cerclage comes off, the baby will come out and need to stay in an incubator. So the doctor recommended staying at the hospital for monitoring again. At that moment, I only thought that my wife and baby must be safe, so we chose not to return to the hospital with treatment rights but to stay at Phyathai 3 Hospital because if my wife was comfortable, the baby would grow well too. It was better than my wife being distressed, which would affect the baby.
Almost reaching the finish line At 30 weeks, the baby’s condition was relatively stable, so the doctor allowed us to go home but advised us to manage nutrition to help the baby gain weight because the baby weighed only 1.2 kg. If the weight did not increase, it would mean delivery was necessary for the baby to grow outside the womb.
At 36 weeks, my wife had labor pains, the cervix dilated further, and the doctor assessed that the baby’s organs were fully developed. Delivery was scheduled for the next day, April 19, 2018. The baby opened his eyes to the world weighing 2,260 grams. It was a preterm birth, but just seeing the baby’s face in the delivery room brought tears to my eyes. When the nurse brought the baby to the nursery room, the baby cried and moved and did not need to be in an incubator. This made me, the father, extremely happy because we managed to keep him in the womb from 21 weeks to 36 weeks, and he was physically ready in every way… I am truly happy.
We share this story hoping that new parents will use our experience to plan during pregnancy. If unexpected events occur, it is important to choose a hospital that is well-prepared with a team of specialized doctors and nurses to provide the best care for the baby and mother.
Words from the treating doctor Dr. Adisorn Aksornphusitpong, Obstetrician specializing in maternal-fetal medicine said that in the case of Mrs. Nattha Phatthawee Suk, the initial risk assessment at the start of prenatal care found no abnormalities. The mother was small and thin, and the father was well-built, so the baby might be small. At 5 months gestation, the mother had diarrhea and improved, but abdominal hardness and a heavy feeling remained, along with cervical dilation, which is like a ticking time bomb that could lead to delivery at any time. The doctor treated with intravenous medication to relax the uterus, placed a cerclage (ideally, the best would be to wait for the cervix to soften and then stitch it, but this was not possible due to advanced gestational age), and monitored the condition. The patient chose to transfer to a hospital with treatment rights.
?After being discharged from that hospital and returning for a checkup at Phyathai 3 Hospital, the doctor found the cervix dilated to the size of the cerclage and recommended hospitalization to prolong the pregnancy while continuously monitoring the baby’s health. Steroid injections were given at appropriate times to stimulate lung development, along with investigating the true cause of cervical dilation. At 36 weeks, the cervix dilated further with labor pains, and since the baby’s organs were ready, delivery was decided. But this family, like many others, felt fear and anxiety?
Understanding the anxiety Dr. Adisorn added that every couple worries when told that preterm delivery is necessary because they want the baby to stay in the womb as long as possible for better health and to avoid the costs and risks of incubator care. The doctor always says, “Yes, it’s better,” but must consider the safety of both mother and baby. Prolonging the pregnancy may cause problems for the mother because all medications have side effects, and severe cases can cause pulmonary edema or heart failure, which can be fatal. Therefore, every decision is carefully considered for the safety of the mother and the readiness of the baby.
Can preterm labor be prevented? Preterm labor can be detected early and managed. For pregnancies without risk, cervical length can be checked. If the cervix is short or the patient is at risk, such as having a short cervix, previous cervical surgery, thin body shape, advanced age, uterine fibroids, or multiple pregnancies, prevention can be done by stitching, tying, or placing a cerclage on the cervix. Treatment methods must be individualized. Another important factor is being under the care of a specialist obstetrician because medication regimens often need adjustment at various levels and with multiple drugs to suit the patient. Specialists can manage these details well. It is also advisable to choose a hospital equipped with comprehensive equipment and technology, Dr. Adisorn said.
