Since most patients at the Bone and Joint Institute are elderly with multiple comorbidities requiring close care, we emphasize the importance of Disease Rounds, small meetings involving doctors, nurses, and medical staff to discuss and consult on high-risk diseases to monitor and evaluate treatment for improvement while the patient is still hospitalized. For complex and difficult cases, we organize Grand Rounds, gathering an experienced multidisciplinary medical team to discuss and share opinions on patient care in complicated cases with the orthopedic attending physician, always prioritizing the best interest of the patient.

Because orthopedic surgery is a very delicate matter but has a significant impact on the body, the Bone and Joint Institute at Phyathai 2 Hospital recognizes this delicacy and will not allow even the slightest error. Therefore, we have designed an important step called the preoperative risk assessment index, or Phyathai Co-Morbid Index. This is one of the excellence services we proudly provide for you.
This risk assessment index… is born from the accumulated experience of doctors
To prevent risks in high-risk patients and achieve the best treatment outcomes, Prof. Dr. Thaninith Chotanaputi, Head of the Bone and Joint Institute and an experienced orthopedic surgeon for over 20 years, together with the medical and nursing teams, developed a quality control program called Phyathai Co-Morbid Index, which is a Quality Service we apply to all patients undergoing bone surgery.
“The principle is that we assess patients using a three-part evaluation conducted jointly by the medical and nursing teams. If the total score is 6-7 points, it is considered safe to proceed with surgery immediately. However, if the total score is 8 or higher, the patient is considered at risk and will enter the MDT Conference (Multi-Disciplinary Conference) process. This involves a multidisciplinary medical team, nursing staff, and relevant personnel meeting to evaluate options and the impact of each option so that the patient and their relatives understand and foresee the outcomes before making a decision. The Co-Morbid Index was designed because Phyathai 2 Hospital wants every patient’s treatment outcome to be as perfect as possible. Therefore, if a patient is found to be at risk, the MDT Conference, consisting of the attending physician, internist, nurses, OPD team, ward team, and nurse case manager, will discuss the issues based on JCI standards regarding risks and success rates. The team will then summarize and inform the patient, relatives, and family about the level of risk, the options available, what will happen if surgery is not chosen. If the doctor considers the patient at risk but does not discuss with relatives or hold a medical team meeting, Prof. Dr. Thaninith will strictly not approve the surgery schedule. This is the doctor’s initiative to ensure everyone prioritizes patient safety above all.
