Breast implant augmentation is a dream of young women to try once in their life to boost their self-esteem and body beauty. But some may hesitate about the results, side effects or recovery process after the surgery. Here are 10 important things you have to know and understand…before making decision to get breast implant surgery.
No. 1 “what do you want?”.
Before making decision, ask yourself first whether “What do you want?” or “What is your problem?” and you will receive the answers why you should get breast augmentation surgery.
Mostly, there are 2 problems that make women decide to get it.
- Having small breasts unsuitable to their body. For example, some women are tall with broad shoulders but have the disproportionate breast size, Cup A. Thus, they decide to get breast implant surgery to fix it. The average age of this group is around 20-30 years old.
- Restoring breast size and shape after pregnancy. Normally, after breastfeeding for a while, the breasts will atrophy down and sag unappealing. To get breast augmentation surgery will help their boobs achieve better shape beautifully.
No. 2 Any breast disorders?
Before making decision to get breast augmentation surgery, we have to know first that besides external appearance, are there any problems inside our breasts? Any cysts or pain? If we’re unsure or have any doubts about tumor or cancer, we should seek for doctors’ examination and treatment before thinking of breast augmentation.
There are many types of breast examinations such as using the pads of our fingers to make breast self-examination or getting a physical examination using X-Ray, mammogram or ultrasound.
No. 3 Differences of each silicone type
Nowadays, we can search about silicone implants from the internet but if you want to get reliable information to compare advantages with disadvantages of each silicone type, you should consult directly with a surgeon.
Classification of silicone types
- Differentiate by the thing filled inside silicone shells such as Saline and Silicone Gel
- Saline filled implant is a medical prosthesis to place a blank silicone shell inside the breast. There is a tube connected to the outside to direct sterile saline solution into an elastomer silicone shell. For example, if we choose the shell size 300cc, after implant operating and placing the shell at the right position, saline will be filled via the tube up to required portion. The silicone shell will be inflated similar to silicone gel. Then the tube will be taken out and the shell will be closed.
- Silicone gel-filled implant is a silicone shell filled with a medical grade gel silicone. It is soft, adjustable and very sticky capable to change shape according to the compression force.
- Differentiate by the textures; smooth surface and rough surface (sandy surface)
- Smooth surface is very clear and smooth.
- Sandy surface is produced to solve the problem of fibrosis after breast augmentation surgery. In the past, breasts became harder after operation for 6 months due to the smooth-surfaced implants causing fibrosis and hardening of the breast tissue which had not happened in the beginning after surgery completion. Therefore, the rough surface like sandpaper is developed to reduce the fibrosis. However, both textures are currently used depending on surgeon’s opinion and patient’s need.
- Differentiate by the shape of silicone; Round and Teardrop
- Round shape will be round for the whole shell like a movable ball without any problems in long run and the surgery is easier than the teardrop shaped silicone.
- Teardrop shape is designed to give the most natural look to the breast as the top is flat and the bottom is sagging which is more popular now. However, the surgery and care are more complicated than the round shape augmentation. A highly skilled surgeon is recommended as it can be flipped over and may need further surgery for any problems or require the massage to make it stay accurately.
No. 4 Choose appropriate silicone size
Some may think that as they will get pain from the surgery so let choose the bigger one. In fact, we have to consider our body before selecting the silicone size as the inappropriate size can lead to many side effects as follows.
- The excessive breasts can make you feel tight and trouble breathing, as well as back pain.
- Having nipple numbness due to large silicones squeeze nipples and nerves
- The larger the silicone the greater the possibility of the hardened breast and the increased fibrosis.
The way to choose the suitable silicone size for your body is to measure chest size, high line in the middle of the chest and chest height from nipple to the base. Then, open the catalogue to choose the appropriate one. The other method is to wear the bra in the cup size needed and put in the silicone required. Look at the mirror whether you like your body now. After that, you can consult with a surgeon using computer imaging technology to help check whether your chosen size is suitable.
No. 5 Incisions Points affect self-recovery.
There are 3 locations of breast surgical incision, these include:
- Under the armpit (Trans-axillary), it’s suitable for Thais because Asian mostly have incision, longer recovery time and possibly keloid much more than others after a surgery. The incision can be hidden under the armpit. However, in the beginning of the recovery, patients will feel very pain as it is adjacent to arm muscles so the movement is limited.
- In the crease under your breast (Infra-mammary), the incision in this area is suitable for foreigners and people whose incision heals quickly without any keloids. It’s easier for both surgery and placing accurately than other locations and very popular recently. However, people having long recovery may have an incision due to the edge of the bra.
- Around your nipple (Peri-areolar), it is made where the darker skin of the areola meets the lighter skin of the breast. It’s hardly seen after healing. The surgery is easy like under the breast (Trans-axillary) and the incision is hidden well. The disadvantage of this location is nipple numbness happening to some patients as the operation is very close to the nipple and may disturb the pectoral chest.
No.6 Location to place silicone
There are 2 positions to place silicone, Under or Over the pectoral muscle
To place under the pectoral muscle (Subpectoral) is suitable for women with small amount of breast tissue. Surgeon will place the silicone under the pectoral muscle and use breast tissue to cover it and prevent the hardened breast. After surgery, there will be more pain and difficult to lift arms.

No. 7 Carefully choose a surgeon… Safety is priority.
Once you know how to choose the suitable silicone and size, the most important priority is safety. A breast augmentation surgery is a major operation using both of anesthetics and doping before implantation. Therefore, it has to take place in a qualified operating room by specialist surgeon only.
No. 8 Self-preparation before breast implants
Prior to breast augmentation surgery, patients have to prepare themselves similar to other major operations by following the procedures below.
- To get physical examination with no harmful underlying disease against an operation such as the level of high blood pressure being in a good range capable to receive anesthetics.
- Blood test result should be normal.
- Before a major surgery, no food and drink 12 hours
- To take menstruation medication to postpone the menstrual cycle due to heavy bleeding during surgery
- Alcohol and smoke cessation for 1 week before surgery to avoid any complications after operation
- To stop certain medications, supplements or vitamins that affect the clotting process
No. 9 Self-Care after Surgery
- Normally, it will take a week before removing stitches
- To massage your breasts regularly to make silicone placement accurately and no harden breasts due to shrinking of fibrosis
- Be careful about Lymphatic congestion or old blood in the breast.
- Be careful about infection, no hard work, no exercise or using a lot of energy as the incision can be torn and infected.
No. 10 Good self-care can last silicone over 10 year.
To choose the silicone and specialist surgeon will make breast augmentation surgery safe and last long as the specialist surgeon can advise you the qualified and appropriate silicone. Normally, the silicone factory will guarantee their medical grade silicone for 5 years only. After the follow up check and there’s no problem, patients will not be required to get any further surgery to adjust or change the silicone. As a result, if you take care yourself well, silicone can last over 10 years.
