For women, age 30 or older is not the sole determinant to begin testing. The initiation of sexual activity serves as a significant indication that it’s time to start paying attention to one’s health and scheduling regular internal examinations. This is because we may be at risk of various gynecological diseases, including cervical cancer. If an internal examination reveals suspicious signs, the doctor may recommend additional ultrasounds. These two steps can detect the majority of abnormalities, covering more than 90 percent of the body. Most importantly, it’s essential to maintain annual check-ups, engage in regular physical activity, and avoid certain foods that can potentially act as hormone accelerators, thereby stimulating the internal system and potentially causing irregularities.
In the past, women often perceived surgery, especially in the uterine area, as a significant event. Concerns included post-surgery wounds, pain, extended recovery times, and potential high costs. However, thanks to ongoing advancements in Thai medicine, undergoing a hysterectomy has become more manageable. These advancements have transformed hysterectomies, evolving from open abdominal surgery to endoscopic procedures involving three or four incisions. Most recently, there has been a development known as single-incision laparoscopic surgery, which has garnered growing interest among women today.
“Single-incision laparoscopic hysterectomy”, also known as “hidden laparoscopic hysterectomy”, has gained popularity internationally. In Thailand, this method has recently emerged, utilizing specialized surgical technology introduced only 1-2 years ago. The key feature of the hidden hysterectomy is that it involves just one incision at the navel, typically measuring 1.2 – 1.5 centimeters in size, discreetly concealed within the belly button. This approach minimizes visible scarring and alleviates post-surgery wound-related concerns, providing female patients with renewed confidence. Additionally, it enables a swift recovery, often within 1 – 2 days, reducing recuperation costs, and facilitating a quicker return to normal work compared to traditional procedures.
Hysterectomy may be recommended for various conditions and symptoms, including those originating from the uterus like uterine fibroids, endometriosis, and severe chronic lower abdominal pain. It can also be considered for conditions related to ovarian cysts, such as chocolate cysts. In cases where the patient wishes to retain the uterus, the surgeon may perform a procedure to remove only the fallopian tubes.
While single-incision hysteroscopy presents a promising alternative for women requiring surgery, its suitability may be limited in certain cases or at the discretion of the physician. For instance, when the size of the uterus exceeds two-thirds of the pelvic floor or when significant fascia is present in the pelvic floor, alternative surgical approaches such as open abdominal surgery or endoscopic surgery with three or four incisions may be more appropriate. The decision will be made by the obstetrician based on the unique circumstances of each patient.
Post-Surgery Advice
After surgery, patients should avoid lifting heavy objects and refrain from sexual intercourse for approximately one month. However, they can resume their regular diet and daily activities.
