Take control of your long-term health with ‘ALL YOU CAN CHECK’, a customizable health screening program designed to support continuous preventive care throughout the year. This program allows you to check more than 70 essential health risk markers, covering chronic diseases, heart health, kidney and liver function, cancer screening, osteoporosis evaluation, and anti-aging assessments. All screenings are performed with advanced medical technology and interpreted by specialist doctors who provide personalized recommendations tailored to your health goals. Your care follows a continuous model of Check, Adjust, Follow-up, and Ongoing Care, ensuring that your results are monitored and optimized throughout the year. The focus is on prevention, early detection, and long-term well-being.

Terms of Service

1. Available for purchase from 8 September 2025 to 31 August 2026.

2. Service Period & Usage Conditions

2.1) Basic Check-up
The first Basic Check-up visit must be completed by 31 August 2026.

2.2) Basic Check-up – Unlimited Items
For items that allow unlimited visits, follow-up visits will be scheduled at the physician’s discretion.

2.3) Advance Check-up Categories
Advance Check-up items can be accessed only after completing the Basic Check-up.

2.4) Program Validity
The program is valid for 365 days (1 year) from the date of the first Basic Check-up.

2.5) Continuous Service Usage
Service recipients may continue using the program within 365 days (1 year) according to the number of visits and conditions specified, counting from the first visit date.

2.6) Continuity of Care
To ensure consistent health monitoring, all follow-up visits must be conducted by the same physician who performed the Basic Check-up.

2.7) Missed or Delayed Visits
If the service recipient cannot complete the check-ups within the specified period, the hospital reserves the right not to issue any refund under all circumstances.

3. Program Service Fees

3.1) The program price includes the doctor’s fee and service fee for the 1st and 2nd visit only.
3.2) From the 3rd visit onward, service recipients are required to pay an additional physician report fee, according to the following rates:

• • • For follow‑up in ‘Laboratory Tests category’ only, service recipients are required to pay an additional 300 THB.
    • For follow‑up in ‘X-ray and CT Scan category’ (1–2 items), an additional 500 THB is required.
    • For follow‑up involving Laboratory, X-ray, and CT Scan combined, an additional 800 THB is required.
    • For follow‑up in ‘Anti‑Aging category’ only, an additional 500 THB is required.

3.3) The program price does not include consultation fees from other specialized medical centers. If abnormalities are detected and referral to a specialist is required, service recipients must pay additional physician fees.

3.4) The program price includes the service fees for all examinations specified. For ‘All You Can Check Lite’ program, service fees for 3 visits are included. If service recipients exceed the allotted number of visits, an additional service fee of 380 THB per visit will apply.

4. The program price includes a meal coupon and a health check‑up report booklet, provided during the first health examination visit.

5. Advance Check-up Categories

5.1) If the physician determines that contrast media injection is required to detect abnormalities, service recipients must pay an additional fee for the contrast agent (8,000–15,000 THB, depending on the type and amount used).

5.2) Examinations in the CT Scan category are subject to the physician’s discretion.

5.3) For service recipients with breast augmentation who choose Digital Mammogram with Breast Ultrasound option, additional charges will apply.

5.4) For service recipients who choose the Mental Health Assessment category, kindly schedule an appointment at least 5–7 days in advance at Psychiatry Center (LET’S TALK), Phyathai Phaholyothin Hospital. Tel. Phyathai Call Center 1772 (Press 9 for English) ext.40595–96

6. Changing Advance Check‑up Categories

6.1) If service recipients have selected specific Advance Check‑up categories but have not yet completed all of them, they may change their selected categories within 365 days (1 year) from the date of the first visit.

6.2) Once all selected categories have been completed, service recipients cannot change the categories on the day of the physician’s follow‑up appointment (second visit).

7. For your convenience, kindly schedule an appointment in advance through Phyathai Call Center at 1772 (press 9 for English)(available 24 hours).

8. Once the program has been used, the benefits cannot be transferred or assigned to another person. The same individual must use the program throughout its validity period.

9. This program is valid for health check‑ups and outpatient services (OPD) only.
It cannot be used for inpatient (IPD) examinations or treatments.

10. This program cannot be exchanged for cash, products, or other services, and is non‑refundable under all circumstances.

11. For service recipients who receive a service coupon, kindly present the coupon to staff before each visit to redeem your benefits. If the coupon is not presented on the day of service, the hospital reserves the right to charge the regular health check‑up rate.

12. This program must be used at the hospital branch where it was purchased. Changing the service location to another branch is not permitted.

13. This program is available for Thai nationals and foreigners (expats) who work in Thailand or have a Thai spouse only.

14. The hospital reserves the right to make changes without prior notice.

15. For programs purchased through online channels (phyathai.com or Shopee), service recipients will receive the official receipt on the day of their health check‑up. The receipt will be issued in the name of the individual receiving the service, not in the name of the purchaser.

16. Service recipients can access their health check‑up reports via Health Up mobile application, or choose to receive the report electronically. If a physical report booklet is preferred, the hospital will arrange delivery via postal mail.

Add-on Package Terms & Conditions

1. The price applies to the vaccine cost only and does not include the doctor’s fee or hospital service fees.
2. This add-on package is available exclusively for Thai nationals and foreigners (expats) who work in Thailand or have a Thai spouse, and who have purchased ALL YOU CAN CHECK program.
3. This program can be used exclusively at Phyathai Phaholyothin Hospital.
