Why do you need to wear a retainer after finishing orthodontic treatment?

When the patient has completed orthodontic treatment, it is time to remove all the orthodontic appliances. At that time, the dentist will provide a device called a retainer to maintain the position of the teeth that have been moved to the correct alignment. It also helps prevent the teeth from shifting back (relapse).

After finishing orthodontic treatment, do you have to wear the retainer all the time?

Generally, after removing the orthodontic appliances, the dentist will recommend wearing the retainer all the time, including while sleeping, and only removing it during meals and tooth brushing. It should be worn continuously for about 6 months because during this initial period, the tissues around the tooth roots will restructure to hold the teeth firmly in place. After 6 months, the dentist usually advises reducing the retainer wearing time to about 10 hours per night, without needing to wear it during the day.

The duration of retainer use may vary for each individual, and the dentist will determine the appropriate time based on the initial alignment of the teeth before orthodontic treatment and the balance of forces from the tongue, lips, and cheeks, which are factors that determine the stability of the tooth position.

Once the teeth alignment is stable, the dentist will gradually reduce the duration and frequency of retainer use. Patients can also observe themselves; if wearing the retainer feels tight around the teeth, it may indicate tooth movement, and the wearing time and frequency of the retainer may need to be increased.

Retainer cleaning and maintenance

Retainers should be cleaned regularly by brushing with a toothbrush and toothpaste after every meal. Every 2-4 weeks, they should be cleaned using effervescent tablets designed for cleaning dentures, which can be purchased at pharmacies or dental clinics. Soak the retainer in water with the tablet for a while to clean areas that the toothbrush bristles cannot reach, reducing tartar and bacterial buildup.

Every time the retainer is removed, it should be placed in a flat plastic case that is easy to carry. Do not wrap the retainer in tissue paper as it is often forgotten and lost. Also, do not put the retainer in a handbag or pants pocket without a case, as this can cause the retainer to bend or become damaged.
